Background: Breathlessness is a multifactorial problem generated through multiple pathways and thus lends itself to a range of interventions, each targeting a specific mechanism. The aging process affects respiratory function and decreases lung elasticity and respiratory muscle strength, which lead to reduced forced vital capacity. Moreover, many conditions, such as chronic obstructive pulmonary disease (COPD) can cause breathlessness among these patients. There is no single intervention to relieve breathlessness, especially when there is chronic refractory breathlessness.  This study reviewed the literature on non-pharmacological treatments of breathlessness management in older patients.
Methods: This review study used the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) analysis to select the appropriate resources. Two journal databases of CINAHL and MEDLINE were used. The inclusion criteria were as follows: original articles available in full text and peer-reviewed articles written in English from 2008-2018, articles focused on non-pharmacological interventions, and the study participants or patients were older people (³ 65 years old) with dyspnea or breathlessness.    
Results: Five articles met all the criteria. It was found that non-pharmacological treatments such as Acu-TENS, cognitive behavior therapy, inspiratory muscle training, and yoga have been used. Most of these interventions had been applied in older patients with COPD, following by thoracic malignancy, and lung cancer.
Conclusion: Based on the results, these non-pharmacological treatments have shown potential benefits in improving breathlessness for older patients. However, other issues such as comorbidities, smoking history, and the size of the sample should be taken into consideration.
